Improve Health in SWVA: Community Themes and Strengths Assessment

Please consider taking this 10 minute Community Themes and Strengths Assessment survey to improve health in Southwest Virginia! The survey aims to identify strategic issues and inform the formation of goals and strategies to improve health in the region. Gathering input from community members throughout Southwest Virginia, this survey will be used to develop a strong understanding of community concerns, strengths, assets, and perception of quality of life. The Blueprint for Health Improvement and Health-Enabled Prosperity, developed in 2009, in partnership with the Southwest Virginia Health Authority, outlined 20 goals and objectives to improve the health of the region. The Blueprint has galvanized the region into action, increasing awareness and progress toward improving population health. Beginning in 2015, the Southwest Virginia Health Authority is supporting an update of the 2009 Blueprint for Health Improvement and Health-Enabled Prosperity. The Healthy Appalachia Institute at The University of Virginia’s College at Wise is working with the Virginia Department of Health and regional partners to update the regional health blueprint and support sustainable change for health improvement in Southwest Virginia.
